Sammanfattning : The epicardial adipose tissue (EAT), which surrounds the heart and is separated from other tissues only by the thin layers of the pericardium, has enjoyed intense research for the past decades. Increased inflammatory activity in the EAT in coronary artery disease has, together with associated volumetric expansion of the EAT and changes in its radiodensity supported theories that the EAT might play a role in the pathogenesis of coronary atherosclerosis or mirror its progression better than traditional risk factors. Sammanfattning : This thesis focuses on the development and testing of electrocardiographic (ECG) methods to diagnose and characterize myocardial ischemia, infarction and fibrosis (scar). The goal is to provide improved decision support to physicians regarding the optimal selection of patients for two costly, invasive therapies – percutaneous coronary intervention (PCI) for patients with acute myocardial infarction (MI) and implantable cardioverter defibrillators (ICDs) for patients with heart failure and reduced left ventricular ejection fraction (LVEF). Sammanfattning : Research questions: 1 Is hypertension and diabetes associated with the future risk of death, and recurrent manifestations of cardiovascular disease in acute coronary syndromes (ACS)? 2 How does myocardial ischemia affect depolarization and repolarization of the heart during the early phase of an acute myocardial infarction? 3 What is the prognostic value of accepted vectorcardiographic markers in relation to future risk of sudden cardiac death after ACS? Methodology: Papers I, III and IV studied patients with ACS prospectively and consecutively included at the coronary care unit of Sahlgrenska University Hospital.
