    Sammanfattning : Internationally as well as in Sweden, efforts for improvement in gynaecological surgery in recent decades have mainly focused on three new treatment concepts:(1) Use of minimally invasive procedures: since there is an interdependency between the extent of surgical trauma and the risk for adverse outcome, increased use of supposedly atraumatic endoscopic procedures has revolutionized several aspects of surgical care(2) A multimodal approach to eliminate harmful procedures in the peri-operative process based on evidence-based principles(3) Introduction of implants to support damaged tissue with synthetic mesh in incontinence and pelvic organ prolapse patients.Research question 1: Is introduction of a minimally invasive operation enough per se or is the measured improvement mediated by elimination of harmful procedures in the perioperative process?Findings: Our study (Paper I) indicates that by applying a multimodal intervention programme for the pre- and postoperative care of patients undergoing supravaginal hysterectomy, the surgical procedure per se is of less importance than generally considered.     Sammanfattning : Objective: To assess the use of cystodefecoperitoneography (CDP) in clinical and radiological diagnosis of cystocele; to study the correlation between clinical and radiological findings at CDP and to evaluate if anatomical topography and structural abnormality corresponds to symptomatic presentation when assessing posterior vaginal wall prolapse; to evaluate if xenograft used at rectocele repair was associated with adverse clinical and histological inflammatory reactions; to prospectively examine if rectocele repair using xenograft is a safe method providing satisfactory clinical and symptomatical outcome; and to investigate the prevalence and risk factors of genital prolapse and urinary incontinence in female patients operated for rectal prolapse. Methods: The following methods were used: The Pelvic organ prolapse quantification system (POPQ), CDP, bowel function questionnaires, pelvic floor surveys, histological, histopathological and clinical inflammatory quantification.     Sammanfattning : The objective of this thesis was to increase the understanding of symptoms related to pelvic support defects, to study the influence of non-obstetric risk factors for pelvic organ prolapse, to describe short-term natural history in women with symptomatic prolapse and to evaluate outcome after surgery. In a cross-sectional survey investigation of the age-specific prevalence of prolapse in Stockholm 8,000 women, 30-79 years of age, were randomly selected from the Swedish Population Register.     Sammanfattning : The objectives of this thesis were to assess the objective, clinical, and sexual outcomes after transvaginal surgery for pelvic organ prolapse using a trocar guided mesh kit, to ascertain the vaginal in vivo histological inflammatory response to large mesh, and to identify possible risk factors associated with exposures after transvaginal mesh surgery.     Sammanfattning : Objective: To determine the frequency of concomitant colorectal disorders in patients with rectocele, and to assess any such association with rectocele; to investigate if paradoxical sphincter reaction (PSR) is influenced by the rectal filling volume; to investigate if surface electromyography (EMG) is a reliable method for diagnosing PSR; to evaluate the use of defeco-peritoneography (DP) in patients with an unexplained widening of the rectovaginal space at defecography; to study the long-term results after rectocele repair and to evaluate parameters that might influence the outcome; to evaluate pre- and postoperative findings at cysto-defeco-peritoneography (CDP) in patients with genital prolapse. Methods: EMG of the anal sphincter muscles was carried out and PSR was diagnosed using needle, wire and/or surface electrodes. LÄS MER
