  1. 6. Improving the Quality and Safety of Drug Use in Hospitalized Elderly : Assessing the Effects of Clinical Pharmacist Interventions and Identifying Patients at Risk of Drug-related Morbidity and Mortality

    Författare :Anna Alassaad; Håkan Melhus; Margareta Hammarlund-Udenaes; Johan Sundström; Claes Mörlin; Lene Juel Kjeldsen; Uppsala universitet; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; Adverse drug events; inappropriate prescribing; medication errors; polypharmacy; medication reconciliation; medication review; patient drug counseling; multi-dose dispensed drugs; risk-estimation; multiprofessional collaboration; prediction model; quality measure; rehospitalization;

    Sammanfattning : Older people admitted to hospital are at high risk of rehospitalization and medication errors. We have demonstrated, in a randomized controlled trial, that a clinical pharmacist intervention reduces the incidence of revisits to hospital for patients aged 80 years or older admitted to an acute internal medicine ward. 2. 7. Drug utilization studies as support to decisions in drug policy in Estonia

    Författare :Raul-Allan Kiivet; Karolinska Institutet; Karolinska Institutet; []
    Nyckelord :drug utilization; Estonia; Nordic Countries; international comparison; antibiotics; neuroleptics; ulcer disease; in-patients; prescriptions; health policy;

    Sammanfattning : The main purpose of the present studies was to describe the use of drugs in Estonia in an international perspective and to study factors determining changes in drug utilization. An important aim of evaluating drug utilization in Estonia was to use the data as a management instrument in areas where initiatives should be taken towards improved use of drugs. LÄS MER

  3. 8. Drug utilization 90% : using aggregate drug statistics for the quality assessment of prescribing

    Författare :Björn Wettermark; Karolinska Institutet; Karolinska Institutet; []
    Nyckelord :Drug sales statistics; drug utilization; feedback general practitioners; guidelines; indicators; intervention; prescribing; quality indicators;

    Sammanfattning : Introduction: There is room for improvement in prescribing. Although a range of indicators have been developed to assess the quality of prescribing, none of them focus on all drugs and there is a gap between what is credible for health care professionals and what can be achieved with the readily available data. 4. 9. Pericyte Influence on Drug Delivery Across the Blood-Brain Barrier : Implications for Therapy of Neurodegenerative Diseases

    Författare :Nebojsa Mihajlica; Margareta Hammarlund-Udenaes; Christer Betsholtz; William Elmquist; Uppsala universitet; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; Blood-brain barrier; pericytes; pericyte-deficiency; pharmacokinetics; small-molecular drugs; drug distribution; transporters; P-glycoprotein; tyrosine kinase inhibitors; Farmakokinetik och läkemedelsterapi; Pharmacokinetics and Drug Therapy;

    Sammanfattning : The blood-brain barrier (BBB) represents a complex interface between the brain parenchyma and systemic blood circulation, strictly controlling exchange of substances between the two sites. Pericytes are mural cells located on the abluminal membrane of the brain endothelium, involved in BBB formation and maintenance. LÄS MER

  5. 10. Pharmacometric tools to support translational drug development
