1. The role of fibroblast growth factor-23 in chronic kidney disease-mineral and bone disorder
Sammanfattning : Fibroblast growth factor-23 (FGF23) was initially identified as the causative factor of autosomal dominant hypophosphatemic rickets. Further studies confirmed that FGF23 is predominantly expressed in the osteocytes and osteoblasts of bone and that circulating FGF23 acts on the kidney to inhibit renal phosphate reabsorption and 1,25(OH)2D3 hydroxylation. 2. The relationship between reduced renal function and cardiovascular disease
Sammanfattning : This thesis examines the relationship between measures of renal function and cardiovascular disease (CVD) in patients with hypertension (the NORDIL study) and in the healthy population (Malmö Diet and Cancer Study, MDC) and whether antihypertensive treatment response and the risk of CVD is affected by genetic variation of a regulator of the renal amiloroide sensitive sodium channel (NORDIL). In study 1 we tested creatinine, estimated glomerular filtration rate (eGFR) with Cockroft-Gault (CG) and the MDRD equations and also microalbuminuria (MA) as predictors of CVD, stroke and CVD death. 3. Fibroblast growth factor-23 and Klotho in bone/mineral and parathyroid disorders
Sammanfattning : Fibroblast growth factor-23 (FGF23) is a novel, bone-produced hormone that regulates renal phosphate (Pi) reabsorption and calcitriol metabolism. Disorders of mineral and bone metabolism, such as autosomal dominant hypophosphatemic rickets (ADHR) and hyperostosis-hyperphosphatemia syndrome (HHS), witness the importance of well-balanced serum levels of FGF23. 4. Genetics of Diabetes Subtypes. Characterization of novel cluster-based diabetes subtypes
Sammanfattning : BACKGROUND: Type 2 diabetes (T2D) has been reproducibly clustered into five subtypes based on six-clinical variables; age at diabetes onset, body mass index (BMI), Glutamic acid decarboxylase autoantibodies (GADA), glycated hemoglobin (HbA1c) and insulin secretion and resistance estimated as HOMA2B and HOMA2IR derived from fasting glucose and Cpeptide. 5. Studies on bone and mineral disorders in CKD : determinants and survival implications
Sammanfattning : Disruptions in mineral metabolism occur already at an early stage of chronic kidney disease (CKD) and may eventually lead to a complex of CKD - mineral and bone disorders (CKD-MBD) characterized by alterations of the mass, turnover rate, mineralization and strength of bones, and high risk of vascular calcification, fractures and other complications resulting in worse survival. The aim of this thesis was to increase the understanding of some aspects of CKD-MBD with special emphasis on clinical and laboratory determinants and implications for clinical outcome. LÄS MER
