11. Abdominal compartment syndrome and colonic ischaemia after abdominal aortic aneurysm repair in the endovascular era
Sammanfattning : Abdominal Compartment Syndrome (ACS) and colonic ischaemia (CI) are serious and potentially lethal complications after open (OSR) and endovascular repair (EVAR) of ruptured (rAAA) and intact (iAAA) abdominal aortic aneurysms. The aims of this thesis were to investigate the incidence, outcome, and risk factors associated with ACS (Papers I-III) and to evaluate extraluminal colonic tonometry for postoperative surveillance of colonic perfusion (Paper IV). 12. Intra-abdominal Hypertension and Colonic Hypoperfusion after Abdominal Aortic Aneurysm Repair
Sammanfattning : Colonic ischaemia (CI), Intra-abdominal hypertension (IAH) and abdominal compartment syndrome (ACS) are devastating complications after abdominal aortic aneurysm (AAA) surgery. The aims of this thesis were to study the incidence and clinical consequences of IAH/ACS and the association between CI and intra-abdominal pressure (IAP) among patients undergoing OR for ruptured AAA (rAAA), to compare extraluminal pHi monitoring, with standard intra-luminal monitoring among patients operated on for AAA, and to study the frequency and clinical consequences of IAH/ACS after endovascular repair (EVAR) for rAAA. 13. Liver transplantation in man. Gastrointestinal perfusion and inflammatory response
Sammanfattning : Patients undergoing liver transplantation develop a systemic inflammatory response which eventually continues with development of a multiple organ dysfunction syndrome (MODS). Gastro-intestinal hypoxia or bacterial translocation has been proposed as the "motor" of MODS. LÄS MER
14. Splanchnic circulation in cardiac surgery
Sammanfattning : The studies addressed different techniques of monitoring hepatic and splanchnic blood flow (SBF) during open heart surgery. Calculations of gastric mucosal pH (pHi) using tonometry was performed in 72 patients, and the effect of dopaminergic stimulation on pH1 was studied in 33 patients. LÄS MER
15. Mesentric hemodynamics and effects of angiotensin II during circulatory stress
Sammanfattning : Mesenteric hypoperfusion and ischemia is considered to be deleterious to the intestinal mucosal barrier function and it is suggested that the renin-angiotensin system plays a pivotal role in the regulation of mesenteric perfusion. The present thesis examines the effects of the specific AT1 receptor blocker candesartan on mesenteric hemodynamics during acute hypovolemia and endotoxinemia.
