1. Heart failure : biomarker effect and influence on quality of life
Sammanfattning : Background and aims: Heart failure (HF) is a life threatening condition and optimal handling is necessary to reduce risk of therapy failure. The aims of this thesis were: (Paper I) to examine whether BNP (B-type natriuretic peptide)-guided HF treatment improves morbidity and mortality when compared with HF therapy implemented by a treating physician at sites experienced in managing patients with HF according to guidelines; (Paper II) to investigate how to define a responder regarding optimal cut-off level of BNP to predict death, need for hospitalisation, and worsening HF and to determine the optimal time to apply the chosen cut-off value; (Paper III) to evaluate how Health-Related Quality of Life (HR-QoL) is influenced by natriuretic peptide guiding and to study how HR-QoL is affected in responders compared to non-responders; (Paper IV) to evaluate the impact of patient age on clinical outcomes, and to evaluate the impact of duration of the HF disease on outcomes and the impact of age and HF duration on BNP concentration. 2. Mineralocorticoid receptor antagonists in heart failure : exploring the gap between guideline-directed medical therapy and real-world practice
Sammanfattning : Heart failure is the possible end-result of a variety of different diseases, where ischemic heart disease and hypertension are the most common causes in high income countries. In Sweden, heart failure has a prevalence of 2% in the adult population and rises to over 10% among people over 70 years of age. The 5-year all-cause mortality is about 50%. LÄS MER
3. Bridging the gap between clinical trials and clinical practice : sacubitril-valsartan in heart failure as a model
Sammanfattning : When novel treatments prove more effective than standard therapies, a swift and effective implementation is needed to reach cost-effectiveness and to benefit eligible patients. Meanwhile, women and elderly are often under-represented in clinical trials, which creates a knowledge gap on how to optimize treatment in clinical practice. LÄS MER
4. PET in Heart Failure - Methods and Applications
Sammanfattning : Positron Emission Tomography (PET) permits regional myocardial perfusion, fibrosis and oxidative metabolism to be non-invasively quantified with radioactive tracers such as [15O]-water and [1-11C]-acetate. 5. Roller skis' rolling resistance and grip characteristics : influences on physiological and performance measures in cross-country skiers
Sammanfattning : The aim of this thesis was to investigate roller ski characteristics; classical and freestyle roller skis’ rolling resistance coefficients (μR) and classical style roller skis’ static friction coefficients (μS), and to study the influence of different μR and μS on cross-country skiers’ performance and both physiological and biomechanical indices. The aim was also to study differences in skiing economy and efficiency between recreational skiers, female and male junior and senior elite cross-country skiers. LÄS MER
