The focus of diabetes care has been to prevent diabetes related complications and thereby reduce mortality. Although chronic illnesses merit a long-term and complex response which would be ideally delivered by a multidisciplinary team of health professionals, most of the current healthcare for chronic illnesses still focus on acute episodes of illness.
