    Sammanfattning : The aim of this thesis was to characterize vascular structure and different aspects of cardiovascular regulation in otherwise healthy subjects with slightly elevated blood pressure and compare to healthy subjects with normotension and also investigate patients with established primary or secondary hypertension and coronary heart disease. Altered cardiac repolarization and arterial baroreflex function has been associated with adverse prognosis and increased risk for ventricular arrhythmias in patients with cardiovascular diseases.     Sammanfattning : Postmenopausal estrogen therapy, initiated early in the menopause, seems to protect against development of atherosclerosis and cardiovascular diseases. This thesis concerns studies of artery wall thickness and arterial stiffness estimated by noninvasive ultrasound techniques in long-term estrogen treated postmenopausal women who initiated therapy at the time of the menopause.     Sammanfattning : In this thesis the common carotid artery intima media thickness in nearly 500 elderly men is independently associated with insulin sensitivity, measured by the gold standard hyperinsulinemic euglycemic clamp test (paper 1), to both COX- and cytokine mediated inflammation independently of other markers of inflammation, oxidative stress and antioxidant status (paper 2) and also to the alleles of the apolipoprotein gene independently of lipid levels and cardiovascular risk factors (paper 3). The echogenicity of the carotid intima-media complex measured as grey scale median predicts mortality independently of common carotid artery intima-media thickness, prior cardiovascular disease and cardiovascular risk factors (paper 4).     Sammanfattning : Background: The development of ultrasonographic arterial imaging, high-resolution B-mode ultrasonography, has created a technical basis for a new diagnostic approach to atherosclerotic disease by offering the possibility of accurate and reproducible quantification of the structural and functional changes in the arterial wall. Flow-mediated dilation (FMD) of the brachial artery and carotid intima-media thickness (IMT) have been shown to be good surrogate markers of atherosclerosis. LÄS MER
