   Sammanfattning : Aims: The overall aim of this thesis was to deepen the knowledge of collaboration in care planning across healthcare provider boundaries for patients with complex care needs. Specific aims were: to highlight specialist nurses´ experiences of coordinated care planning in primary care (I); to investigate care center manager ´s understanding of the ability of primary care to provide coordinated care planning for patients with complex needs (II); to focus on health professionals from different organizations and describe their experiences of obstacles and opportunities for collaboration with patients and their relatives and between caregivers for frail elderly patients with multiple diseases (III); to investigate inter-organisational collaboration on care efforts for patients with complex care needs (IV).    Sammanfattning : The study focus is a sample of women with drug dependence admitted to compulsory treatment according to the Swedish act of care for drug abusers (LVM, 1988:870) and according to the act of care for young persons (LVU, 1990:52). From a cohort consecutively admitted to residential treatment at Lunden between 1997 and 2000, a sample of 132 women was selected for a five year follow-up. LÄS MER

   Sammanfattning : This thesis explores cancer from a primary care perspective covering three areas: diagnosis, posttraumatic stress disorder (PTSD), and end-of-life care. We analyzed patient records of every child diagnosed with a malignancy in a defined area. During 12 years 68 children were diagnosed (incidence 14/100,000). LÄS MER
