  1. 1. Improving the quality of caesarean section in a low-resource setting : An intervention by criteria-based audit at a tertiary hospital, Dar es Salaam, Tanzania

    Författare :Andrew Hans Mgaya; Birgitta Essén; Hussein Kidanto; Lennarth Nyström; Jeroen van Dillen; Uppsala universitet; []
    Nyckelord :Caesarean section; Criteria-based audit; Fetal distress; Obstructed labour; Low resource setting; Robson classification; Obstetrik och gynekologi; Obstetrics and Gynaecology;

    Sammanfattning : A sharp increase in caesarean section (CS) rates at the Muhimbili National Referral Hospital (MNH) – a tertiary referral hospital in Tanzania – by 50% in 2000–2011, was associated with concomitant increase in maternal complications and deaths and inconsistent improvement in newborn outcomes. The aims of this thesis were to explore care providers’ in-depth perspective of the reasons for these high rates of CS, and to evaluate and improve standards of care for the most common indica-tions of CS, obstructed labour and fetal distress, which are also major causes of adverse maternal and neonatal outcomes. 2. 2. Unlocking Potentials of Innovation Systems in Low Resource Settings

    Författare :Julius Ecuru; Blekinge Tekniska Högskola; []
    Nyckelord :SAMHÄLLSVETENSKAP; SOCIAL SCIENCES; SAMHÄLLSVETENSKAP; SOCIAL SCIENCES; Cluster; Innovation; Innovation System; Low resource setting; Research; Science; Technology; Technoscience; Triple Helix; Uganda;

    Sammanfattning : This study examined the dynamics, challenges and opportunities of developing innovation systems in low resource settings with a particular focus on Uganda. It applied perspectives of technoscience and concepts of innovation systems, triple helix as university-industry-government relationships, mode 2 knowledge production and situated knowledges in understanding the context, identifying key policy issues and suggesting ways to address them. 3. 3. Quality of maternal healthcare : Exploring severe maternal outcomes and the influence of delays and decision-making on caesarean sections in a low-resource setting

    Författare :Jonah Kiruja; Marie Klingberg-Allvin; Fatumo Osman; Helena Litorp; Abdi Gele; Högskolan Dalarna; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; Somaliland; maternal mortality; delayed caesarean section; maternal near miss; severe maternal outcomes; emergency obstetric care; low resource setting;

    Sammanfattning : Somaliland has one of the highest maternal mortality rates worldwide, yet there is limited evidence for severe maternal outcomes and challenges with the provision and utilisation of emergency obstetric care (EmOC). This thesis studied quality of maternal healthcare by exploring severe maternal outcomes and the influences of delays and decision-making on caesarean sections in Somaliland. LÄS MER

  4. 4. 'What about the Mother?' : Rising Caesarean Section Rates and their Association with Maternal Near-Miss Morbidity and Death in a Low-Resource Setting

    Författare :Helena Litorp; Birgitta Essén; Hussein L. Kidanto; Elisabeth Darj; William Stones; Uppsala universitet; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; caesarean section; complications; attitudes; women; caregivers; low-income countries; Tanzania; Medicinsk vetenskap; Medical Science;

    Sammanfattning : In recent decades, there has been a seemingly inexhaustible rise in the use of caesarean section (CS) worldwide. The overall aim with this thesis is to explore the effects of and reasons for an increase in the CS rate at a university hospital in Dar es Salaam, Tanzania. LÄS MER

  5. 5. Network models with applications to genomic data: generalization, validation and uncertainty assessment

    Författare :José Sánchez; Göteborgs universitet; []
    Nyckelord :NATURVETENSKAP; NATURAL SCIENCES; Inverse covariance matrix; precision matrix; graphical models; high-dimension; low-sample; networks; sparsity; fused lasso; elastic net; cancer; TCGA pan cancer analysis; online resource; discriminant analysis; classification; networks;

    Sammanfattning : The aim of this thesis is to provide a framework for the estimation and analysis of transcription networks in human cancer. The methods we develop are applied to data collected by The Cancer Genome Atlas (TCGA) and supporting simulations are based on derived models in order to reflect real data structure. LÄS MER
