    Sammanfattning : P-wave morphology reflects the projection of the depolarization vector on the electrocardiogram (ECG)lead axes in three-dimentional space and reflects both right and left atrial activation. The main purpose of current thesis was to explore the internal logic between the atrial conduction properties, including intra- and inter- atrial conduction, and P-wave characteristics from the level of histology to electrophysiology.     Sammanfattning : Research questions: 1 Is hypertension and diabetes associated with the future risk of death, and recurrent manifestations of cardiovascular disease in acute coronary syndromes (ACS)? 2 How does myocardial ischemia affect depolarization and repolarization of the heart during the early phase of an acute myocardial infarction? 3 What is the prognostic value of accepted vectorcardiographic markers in relation to future risk of sudden cardiac death after ACS? Methodology: Papers I, III and IV studied patients with ACS prospectively and consecutively included at the coronary care unit of Sahlgrenska University Hospital.     Sammanfattning : The aim of this thesis was to evaluate the role of interatrial conduction defects in the genesis of lone paroxysmal atrial fibrillation (AF) using combined approach of non-invasive signal-averaged P-wave ECG, invasive electrophysiological (EP) studies, anatomical studies and animal experiments. Non-filtered signal-averaged P-wave ECG revealed irregularities in orthogonal P-wave morphology and the presence of double-peaked configurations of P-wave spatial magnitude in patients with lone paroxysmal AF compared with healthy individuals (I), suggesting the presence of interatrial conduction delays localised in the posterior-inferior interatrial route.
