  1. 1. Laparoscopic or Open Antireflux Surgery - A Comparative Study with Special Reference to the Patient´s Perspective

    Författare :Gunilla Nilsson; Folke Johnsson; Bengt Hallerbäck; Institutionen för hälsovetenskaper; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; Antireflux surgery; fundoplication; RCT; laparoscopy versus open surgery; subjective outcomes; objective outcomes; narratives; short-term follow-up; long-term follow-up; Surgery; Kirurgi; Health Informatics; Hälsoinformatik; Omvårdnad; Nursing; Gastro-enterology; Gastroenterologi; traumatology; orthopaedics; Surgery; long-term follow-up; short-term follow-up; narratives; objective outcomes; subjective outcomes; laparoscopy versus open surgery; RCT; Antireflux surgery; fundoplication; Kirurgi; traumatologi; ortopedi;

    Sammanfattning : The overall aim of this thesis was to compare the effects of laparoscopic versus open antireflux surgery in a randomized clinical trial focusing the patients' subjective and objective outcome postoperatively, from a short-term and a long-term perspective. A further aim was to describe GORD patients' experiences of illness and surgical treatment and if possible to find striking characteristics relating to laparoscopic or open surgery. LÄS MER

  2. 2. A Laparoscopic Approach in Gastro-Oesophageal Surgery : Experimental and Epidemiological Studies

    Författare :Rune Sandbu; Bo Anderberg; Uppsala universitet; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; Surgery; Gastro-oesophageal surgery; fundoplication; laparoscopy; long-term results; pneumothorax; intra-cranial pressure; positive end-expiratory pressure; Kirurgi; Surgery; Kirurgi; Surgery; kirurgi;

    Sammanfattning : The extension of laparoscopic procedures into the chest may induce specific pathophysiologic effects.In pigs, we have demonstrated how devastating a combined thoraco-laparoscopic approach can be for gas exchange. Furthermore, the transmission of elevated pressure intra-cranially is a potential danger. LÄS MER

  3. 3. Reconstruction in the gastroesophageal junction- from routine to advanced

    Författare :Pauline Djerf; Lund Kirurgi; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; Gastroesophageal junction; Antireflux surgery; Esophagectomy; Gastric tube; Epiphrenic diverticula.;

    Sammanfattning : The general aim of the thesis was to contribute to a more evidenced based framework in the surgical treatment of diseases in the gastroesophageal junction (GEJ), by comparing outcome, measured by complication and survival rates, and evaluate different reconstructions regarding long-term symptoms and quality of life (QoL). The GEJ is a poorly defined anatomic area that represents the lower part of the esophagus and its’ junction to the proximal stomach. LÄS MER

  4. 4. The Columnar Lined Esophagus. Pathophysiological and Clinical Aspects

    Författare :Stefan Öberg; Lund Kirurgi; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; Intestinal metaplasia; Barrett s esophagus; Esophageal columnar metaplasia; Metaplasia; Pathophysiology; Gastroesophageal reflux disease; Esophagus; Surgery; orthopaedics; traumatology; Kirurgi; ortopedi; traumatologi;

    Sammanfattning : There is a persisting controversy concerning the definition, etiology and pathogenesis of Barrett's esophagus. The dramatic and parallel increase in the incidence of esophageal and junctional adenocarcinoma over the past decades justifies an evaluation of the pathophysiology of premalignant metaplastic changes of the esophagus and the gastroesophageal junction. 5. 5. Gastroesophageal reflux disease - complications and prognostic aspects

    Författare :Dan Falkenback; Lund Kirurgi; []
    Nyckelord :MEDICIN OCH HÄLSOVETENSKAP; MEDICAL AND HEALTH SCIENCES; Gastroesophageal reflux disease; Barrett s esophagus; natural course; GERD; Adenocarcinoma; prognosis; biomarkers;

    Sammanfattning : This thesis investigates several aspects on gastroesophageal reflux disease (GERD) and associated complications as Barrett’s esophagus and esophageal adenocarcinoma. I. To clarify the natural course of GERD, a pH-metry verified cohort was re-evaluated after 20 years. The course of GERD may well be progressive. LÄS MER
